To save time and prepare for your initial visit, you are welcome to print out and complete the following forms. If you have any questions about the content, please leave blank and we can explain when you arrive.

What you need for your first appointment

1) Insurance card(s)
2) Intake forms (above), otherwise you can fill them out upon arrival
3) Physician referral, if required
4) Any imaging reports (i.e. X-ray, MRI), if available
5) Medication list, if available
6) You!!

What to expect:

Please plan to arrive 10-15 minutes early to your first appointment to fill out necessary paperwork. Completing the intake forms prior to your appointment will help expedite the process. Your initial visit will likely take between 45-60 minutes. Your visit will include some degree of objective assessment and treatment so dress in comfortable, mobile clothing.

Following your evaluation, you’ll leave with confidence knowing that you are in good hands. You should have a good idea of what is causing your problem, what to do about it and what to expect in your upcoming treatments.

Insurance

We accept and are in-network with most major insurance plans. Prior to treatment, we encourage you to call your insurance carrier and verify that we are in your care network. We will also verify your insurance benefits and explain them to you. *Please note that verification of benefits is not always a guarantee of coverage until claims are processed.* This is why we encourage both parties to check benefits to ensure accuracy. We also offer affordable self-pay rates if necessary.
